Clearance

In order to participate in high school sports, all athletes must complete the athletic clearance procedure. While this covers every sport during the year, it must be repeated again every school year.

1. Complete https://athleticclearance.com/

   • Click on CA (California)

   • Create an account.

   • For school, select La Quinta/Westminter (NOT La Quinta/La Quinta)

   • Complete all the required information online.

   • All paperwork, including the Risk Form and COVID consent form, can be turned in online.

2. Have a doctor complete a Physical examination.

   • While the exam itself can be completed within 10 minutes, most doctors take several weeks for an appointment. Please schedule your visit early.

   • The papers needed to signed by the doctor can be found HERE, from the coaches, or at athleticclearance.com.

   • Walk in clinics are available such as the one with information below.

   • The Physical is the only paper form that must be turned in, which can be handed in to the Athletics office or on the first day of practice.
